- Any claims in the last three to five years, with dates, descriptions and amounts paid
- Construction, fire protection and security details for the site
- Sums insured for buildings, containers, contents, stock and theft
- Business interruption figures, if you want that cover
- Your public liability limit and turnover figures
- The name and position of the person signing the declaration
